Neighborhood Health Champion Award Application
The Mayor’s Health & Fitness Council is pleased to announce a new program to recognize individuals making a positive impact on their neighborhood’s health called Neighborhood Health Champion Award.

Thank you so much for all the nominations for this awesome award. We recognize all the individuals making the health of their neighborhoods and communities better.
Tell us who is making a difference in the health of your neighborhood or community – It can be you or someone else.
This award recognizes a person helping other people be healthier. The person being nominated for this award regularly shows concern for the health of their neighborhood and offers service in making the health of their neighborhood better. The person shows this by encouraging healthy eating, physical activity, well-being, and/or tobacco-free living in Austin.
The person has shown any of the following:
- Getting others to help increase healthy living in the neighborhood through tobacco prevention, healthy eating/food access, active living, and/or overall wellness.
- Helping a nonprofit or an ordinary group of people doing a project that had healthy effects
- Using new ideas and creativity to make healthy living easier
- Helping create an environment or encourage situations that make it easier to be healthy
How to Nominate
The Online Survey Monkey version of the application will allow you to nominate online. The link to the application is provided below. Your online application will only be accessible to Mayor’s Health & Fitness Council (MHFC) once it has been submitted (i.e. incomplete applications cannot be accessed by MHFC).
